The 10-step ECG read
Rate, rhythm, axis, intervals and the patterns you must not miss.
Key points
- Rate: 300 divided by the number of large squares between R waves (or count complexes in 10 seconds × 6).
- Rhythm: a P wave before every QRS and a QRS after every P means sinus rhythm.
- Axis: QRS positive in leads I and aVF is a normal axis.
- Intervals: PR 120–200 ms, QRS under 120 ms, QTc under about 450 ms in men and 470 ms in women.
- ST elevation in two contiguous leads with reciprocal depression suggests acute coronary occlusion.
